What it is
MK-677 (ibutamoren) is an orally active, long-acting ghrelin-receptor agonist — unusual for being a non-peptide small molecule that raises GH/IGF-1.
Proposed mechanism
Sustained GHS-R agonism raises GH and IGF-1 around the clock; because it's oral and long-acting it behaves differently from injectable pulsatile GHRPs.1 Water retention and insulin-sensitivity effects are noted.
State of the evidence
Studied in humans (e.g., for body composition, sleep) but never FDA-approved.
